The Objects of Kiwanis International

  Each year the Kiwanis Club of Sand Lake, NY receives many requests from an assortment of individuals and groups for funding assistance for varied programs and projects.  As the club has limited funds, each request must be considered individually by the appropriate club committee(s) and/or the Board of Directors.  To aid the club in evaluating your request we ask that you take a few moments and provide the following information.  To ensure that your request receives the careful consideration that it deserves, the evaluation process may take up to 30 days, please submit your request in a timely manner.  Should you need more room to respond to any question or if you would like to provide the club with additional information please attach additional sheets to this form.
